Leading Oral and Maxillofacial Surgeon
Dr. Amir Chalak was born in San Francisco and raised in Burlingame, CA. He attended the prestigious UC Berkeley for his undergraduate studies where he graduated in 2006 with a degree in Molecular and Cellular Biology and was heavily involved in the Cal Berkeley Pre-Dental Society.
Dental School and Research Achievements
Dr. Chalak moved to sunny Southern California to attend the highly competitive UCLA School of Dentistry. During his time at UCLA, he developed a strong passion for Oral and Maxillofacial Surgery and was very involved with the UCLA Oral Surgery Club. While at UCLA, he gained recognition and awards for his research involvement in salivary diagnostics and his high performance in the field of Oral and Maxillofacial Surgery. He graduated with distinction of magna cum laude from UCLA School of Dentistry in 2011 and was honored by being inducted into the prestigious National Dental Honor Society of Omicron Kappa Upsilon.
Residency and Medical Training
After graduating from dental school, Dr. Chalak was excited to make his way back to the Bay Area for his Oral and Maxillofacial residency training at UCSF. During his residency program, he attended and graduated from UC Davis School of Medicine in 2015. He completed training in general and pediatric anesthesia and General Surgery in 2016. Dr. Chalak was chosen to be a leader in the Resident Association of Oral and Maxillofacial Surgeons representing all residents at the national level. He completed his training as Chief Resident at the UCSF, SF VA, and San Francisco General Trauma Hospitals in 2017.
Dr. Chalak successfully met the requirements and has achieved Board Certification in Oral and Maxillofacial Surgery.

Dr. Chalak strives to exceed the expectations of his patients. His goal is to deliver the highest standard of care and provide ethical-based oral and maxillofacial surgery. The practice is committed to minimizing the associated stress and anxiety from surgery by providing thorough communication of the entire process. He currently has his own practice in Campbell, Cupertino and Los Gatos. He is certified to use the first FDA-approved robot in the US for the placement of dental implants: YOMI.
